We offer free consultations. Please call to book an appointment at 813-872-2696. A consultation is the best way to get an estimate of your surgical costs, but you can call the office and request an estimate.
A Surgery Fee and Payment Contract will be created for each individual at the time of scheduling a surgery. A deposit is required at the time of scheduling. Balance due must be paid 14 days prior to surgery or at the time of the pre-op visit.
Any payment that is returned as non-payment or charge-back will be referred to an outside collection agency if arrangements of payment are not made within 2 weeks of returned payment. Accounts referred to an outside collection agency or an attorney may be subject to a collection fee of 33% – 50% based on the contract with the collection agency, which will be added to the total balance due at the time of write-off.
Additional fees may include: anesthesia fees, garment costs, lab test and other costs depending on the elective procedures requested by patient. Initial standard pre-op labs are billed directly to the facility. If any repeat labs are needed, it is the patient’s responsibility to cover those fees. Any type of pathology sent out will be the patient’s responsibility. The patient can expect to receive an invoice from the laboratory. Please contact the lab directly for estimate, billing practices and participation with your insurance carrier. Of note, since we offer elective surgery, most insurance will not cover any lab work or pathology tests ordered.

Cancellation Policy – All monies paid on the account toward the surgery, less a $300.00 cancellation fee, will be refunded if the surgery is cancelled more than 14 days in advance. After one year, there will be no refunds, resulting in the full loss of the deposit.
